Lewis University Student Complaint

Lewis University is committed to a policy of fair treatment of its students in their relationships with fellow students, faculty, staff and administrators. Students are encouraged to seek an informal resolution of a dispute directly with the faculty or individual(s) involved when possible. For matters where a resolution is not feasible, a student or a Lewis faculty or staff member on their behalf, may make a formal complaint. Other stakeholders (e.g. parent, community member, alumni) may use this form to submit a complaint, providing their contact information in the student information sections. Please be assured that no adverse action will be taken against a student who files a formal appeal or complaint.
